Report Submission Process
As the social worker, you will prepare a written post placement report after each home visit and submit the report to Holt’s Post-Placement Team. In addition to the report, your family will be responsible for submitting photos along with each report. These photos can be given to you during your visit or sent directly to Holt’s Post-Placement Team.
The report and photos are then sent from Holt directly to HSF in compliance with their requirements.
The Thailand Program requires two phases of Post-Placement reporting:
- Upon arrival home – three detailed post-placement reports prepared by you, the social worker, which are to be completed at two, four, and six months after the child comes home. These reports must be accompanied by at least 6-8 photos of the child submitted online by the family.
- Annually – Parents are asked to submit an annual report after arrival through the first 5 years after the family has Registered the adoption at the Thai Embassy with a final family report due when the child turns 18 years of age. This includes the child’s development/activities and 6-8 photos of the child. This self report is uploaded by the family.
Report Timeline
Visits with your family will take place at the following intervals from the date the child arrived in the US. One original report must be mailed to the Eugene Office.
- 2 Months (no sooner than exactly 2 months after your arrival date)
- 4 Months (no sooner than exactly 2 months from the home visit date of your 2 month report)
- 6 Months (no sooner than exactly 2 months from the home visit date of your 4 month report)
Photo Requirements
Families are required to submit 6-8 photographs with all of their reports. Holt cannot forward reports to Thailand without these photos. Remind families to please upload their required photos on Holt’s Post Placement Report webpage.
- Quantity: 6-8 good quality photos for each report
- Quality: Select photos that are clear, centered, and tasteful – no messy faces, partial dress, etc.
- What: Include typical activities, local events, holidays, playing at the park, family outings, etc.
- Who: Some photos must show child WITH parent(s) and other family members, not just alone
- Avoid: Photos in swimsuits, bath time, only partially dressed, messy food faces, etc.
