Report Submission Process
As the social worker, you will prepare a written post placement report after each home visit and submit the report to Holt’s Korea Program Staff in the Eugene Office. To submit these reports, please email 1 signed color copy on agency letterhead to Koreapostplacement@holtinternational.org.
Photo Requirements
In addition to the report, photos need to be uploaded by the family for each report and submitted through the photo uploading system:
Post Placement Reports are not considered complete without the required photos, so we ask that families send us photos prior to the home visit or the same day of the visit to ensure the report’s timely completion. The report, photos, and supplemental materials are then sent from Holt directly to Korea in compliance with their requirements.
⚠️ Prior to your first post-placement visit, you will receive detailed information and instructions from the Korea Travel Coordinator via email. Please adhere to these instructions as the Korean Ministry pays close attention to compliance!
Report Intervals: Families Arriving BEFORE April 1, 2021
Visits with you (the social worker) will take place in the family’s home at the following intervals from the date the child arrived in the U.S.:
- 2 Month – If your state requires a home visit within the first 30 days this must be explicitly stated in the report in order for the Ministry to allow flexibility. Otherwise, the Ministry requires the first home visit to take place 30-60 days after arrival.
- 4 Month
- 8 Month
- 12 Month
Important: The home visit must be completed before the family’s one year arrival date. If this is not completed before the one year arrival date, the Ministry will require a 5th post placement report be completed.
Report Intervals: Families Arriving AFTER April 1, 2021
Visits with you (the social worker) will take place in the family’s home at the following intervals from the custody date. All reports must be written by the social worker within 10 days of the home visit.
- 1 month – Visit must be completed within 30 days of the family’s custody date.
- 2 month
- 4 month
- 6 month
- 8 month
- 10 month
Important: The report must be sent to Korea before the one year anniversary of custody date. Otherwise an additional report is required.
